Assistant Admissions Coordinator
JOB SUMMARY: The Admissions Coordinator is responsible for completing additional outreach attempts to members assigned to the CME and supporting the Admissions Coordinator’s role, which ensures all referrals for Care Coordination through the OhioRISE Initiative are connected to services with Wingspan’s CME. The Assistant Admissions Coordinator is supervised by the Director of Lorain CME.
ESSENTIAL DUTIES:
- Create a warm, welcoming, helpful and professional atmosphere in the office for all clients and staff.
- Responsible for assisting the Admissions Coordinator in creating and maintaining client files in accordance with licensing and accreditation rules. This includes opening new clients in the electronic health record and updating the system when changes of status occur to ensure consistency of database information in relation to individual client record.
- Responsible for continued outreach to clients/families who have been referred to engage in Care Coordination Services.
- Document in client chart all communication and outreach efforts to engage in services.
- Communicate with Admissions Coordinator and Director of CME when families engage and request care coordination. Assisting in the assignment of cases to the Care Coordinators.
- Represent the Agency in the development or maintenance of relationships with other organizations including sister agencies, public agencies, managed care companies, insurance companies, funders and referral sources.
- Responsible special projects as assigned.
OTHER DUTIES:
- Attend scheduled staff meetings, supervision, and on-going training.
- All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
- Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.
- Respect the privacy of clients and hold in confidence all information obtained during the client's treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
- Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
- Other duties as assigned by management.
QUALIFICATIONS:
Education: Minimum High School Diploma required.
